Tretalone: What's In It and How It Works

Tretalone is Nolla’s restorative nighttime formula. It combines three active ingredients - tretinoin, spironolactone, and niacinamide - making it one of the more comprehensive formulas in the Nolla range. If this is your prescription, your clinician has assessed that your acne has both a hormonal driver and a structural one, and that addressing both simultaneously is the most effective approach for your skin.

What’s In Tretalone

Tretinoin is a prescription-strength vitamin A derivative that accelerates skin cell turnover. It prevents dead cells from accumulating inside follicles, stimulates collagen production, and normalises the way follicle cells behave. It’s the most clinically evidence-backed topical treatment for acne and is particularly effective for comedonal acne and long-term skin texture improvement.

Spironolactone is the ingredient that distinguishes Tretalone from Tretorin. Applied topically, it works as an androgen blocker - it reduces the effect of androgens (hormones like testosterone and DHT) on the sebaceous glands, which directly lowers sebum production. Hormonal acne is typically characterised by breakouts that are cyclical, concentrated around the jawline and chin, and often painful and cystic. Topical spironolactone achieves sebum regulation without the systemic hormonal effects of oral spironolactone.

Niacinamide reduces active inflammation, reinforces the skin barrier (which tretinoin disrupts during the adjustment period), and inhibits melanin transfer - targeting the dark marks that hormonal and cystic breakouts tend to leave behind.

What To Expect - And When

Weeks 1–3: The tretinoin component means purging is expected - existing congestion surfaces faster than it would naturally, leading to a temporary increase in breakouts. Dryness and mild irritation are also common. This phase is the adjustment period, not an indication the formula is wrong for you.

Weeks 4–6: Purging should settle. The combination of tretinoin and spironolactone takes several weeks to shift the skin’s sebum production, so changes to the deeper hormonal driver of acne become more apparent in this window.

Weeks 8–12: Fewer active breakouts, less severe cystic spots, and improved skin texture are typical with consistent use by this point. The hormonal regulation from spironolactone is particularly noticeable over this timeframe.

How To Use It

Tretalone is a nighttime formula - apply it in the evening after cleansing and before moisturizer.

  • SPF every morning without exception. Tretinoin significantly increases UV sensitivity. Skipping SPF actively undoes the overnight repair work.
  • Don’t abandon the formula early. The first three weeks are the hardest. Most patients who stop early do so right before the improvement phase begins.
  • Moisturise after application. A non-comedogenic moisturiser applied after Tretalone each evening significantly reduces irritation.
  • Every-other-night application is fine to start. If irritation in the first two weeks is significant, apply on alternate nights before building to nightly use.
  • No other actives in the same evening routine. No AHAs, BHAs, vitamin C, or additional retinoids alongside Tretalone.
  • Avoid during pregnancy. Retinoids are contraindicated in pregnancy - flag any change in circumstances through the app immediately.

Who Tretalone Is Designed For

Tretalone is specifically suited to hormonal acne patterns: cyclical breakouts, cystic or painful spots concentrated around the jaw and chin, and acne that fluctuates with the menstrual cycle. It’s also appropriate where skin texture and post-acne pigmentation are significant concerns alongside active acne.

The Bottom Line

  • Tretalone combines three actives: tretinoin (cell turnover), spironolactone (hormonal sebum reduction), and niacinamide (inflammation and barrier support)
  • Spironolactone targets the hormonal driver of acne - particularly useful for cyclical breakouts around the jaw and chin
  • Purging and adjustment in weeks 1–3 is expected; meaningful improvement typically appears at weeks 8–12
  • Daily SPF and nightly moisturiser are both essential with this formula
  • This is a comprehensive formula for complex hormonal acne - it requires patience but addresses multiple drivers simultaneously
